Injury
Reporting:
Significant injuries such as fractures, ligament
tears, or serious
concussion have not been frequent the past 10 years in Region 10. A
player'
s registration form serves as emergency treatment authorization if
necessary. Any injury requiring medical evaluation or treatment
should be
reported to the Safety Director as soon as possible. He will send
you a
Parent Letter
that explains the AYSO insurance claim process.
The Accident Report
Form is available for coaches, referees, other AYSO officials,
or parents to report an injury. It is not necessary to use this
form. You may contact the Safety Directory by e-mail, fax, pager, or
phone to report the injury. The Safety Director will assist you in
filing the necessary
AYSO Notification of Injury form and explain when a
Participation Release Form is necessary.
Minor injuries such as bruises and twisted ankles that improve
overnight and permit returning to normal activity the next day do
not need to be reported. Please feel free to contact the Safety
Director for assistance with any soccer injury or health concern.
